Referrals Process

Referrals to Homestyle Care can come from a wide variety of backgrounds including placement breakdowns, residential colleges, community support units and individuals.

Each person referred to Homestyle Care is treated as an individual whose needs, abilities and personality are assessed in detail in order to determine their suitability for placement within our home. Referrals can be made by a Social Worker/Care Manager or directly from families themselves. However, before assessment can take place the Care Manager acting on behalf of the service user must be consulted.

The referral process also includes inviting family, friends and Care Manager to visit the home to assess for themselves the environment and services available. Ideally the service user would also visit the home in person but if this is not possible we recommend a visit by someone who knows the individual well and can act on their behalf.

A transition plan, tailored to the service user’s individual needs, can then be put in place to provide as much support as possible in the move to their new home.
